807 Transportation and Priviate Vehicles – Volunteer Automobile Driver Authorization Form
This form must be on file at the Langham Elementary School before children will be allowed to travel with adults other that their parents/guardians to school functions.
As well, drivers must have on file a current (within 6 months of submission) Criminal Record Check, including a Vulnerable Sector Check, and sign a Declaration of Understanding document.
Please go to the above link, complete the form, and return it to the school along with the necessary record check documents.
