The best way to fill out the form is to download it to your computer if possible. Once downloaded, fill out the form and save it (Ex. “Jane Doe Registration”). Note: A typed signature is acceptable. You do not have to print of the form and sign it. After completed and saved send the registration document through your email as an attachment. Email completed form to firstname.lastname@example.org or email@example.com

The bus request form does require you to print it off in order to draw a map to your pick up. Completed forms can be scanned or a picture taken and emailed to firstname.lastname@example.org or email@example.com
